【志贺氏菌】


细菌性痢疾(bacillary dysentery)简称菌痢,亦称为志贺菌病(shigellosis),是志贺菌属(痢疾杆菌)引起的肠道传染病。志贺菌经消化道感染人体后,引起结肠黏膜的炎症和溃疡,并释放毒素入血。临床表现主要有发热、腹痛、腹泻、里急后重、黏液脓血便,同时伴有全身毒血症症状,严重者可引发感染性休克和(或)中毒性脑病。菌痢常年散发,夏秋多见,是我国的常见病、多发病。儿童和青壮年是高发人群。

Shigella


Bacterial dysentery (bacillary dysentery), also known as shigellosis, is an intestinal infectious disease caused by Shigella spp. After ingesting the human body through the digestive tract, Shigella causes inflammation and ulceration of the colonic mucosa and releases toxins into the blood. The main clinical manifestations are fever, abdominal pain, diarrhea, tenesmus, mucous pus and bloody stools, accompanied by symptoms of systemic toxemia, and severe cases can cause septic shock and (or) toxic encephalopathy. Bacterial strains are distributed all year round and are common in summer and autumn. They are common and frequently-occurring diseases in China. Children and young adults are high-risk groups.


Etiology

Pathogenic bacteria

The pathogenic bacteria that cause bacterial dysentery are Shigella, also known as Shigella, belonging to the genus Shigella of the Enterobacteriaceae family. It is a facultative anaerobic gram-negative bacillus with pilus, flagella, decidua, and teeth. Power is best suited for aerobic growth. Shigella were classified into 4 groups (Shigella dysenteriae, Shigella flexneri, Shigella meningitidis, Shigella sonnei) and 51 serotypes according to different antigen structures and biochemical reactions. At present, China is dominated by Freund's and Shigella sonnei, and Shigella is still endemic in some areas.

Whether or not Shigella enters the body is related to the number of bacteria, pathogenicity and resistance of the human body. Shigella dysenteriae has the strongest virulence and can cause severe symptoms. Most Shigella spp. infections were atypical. Shigella flexneri infection can easily become chronic. As long as 10 to 100 bacteria enter the human body, Shigella, which is highly pathogenic, can cause disease. Some chronic diseases, over-fatigue, overeating and other factors can lead to a decrease in the body's resistance and facilitate the invasion of Shigella.

Shigella invades the intestinal mucosal epithelial cells and the intrinsic layer, causing inflammatory reactions and small blood vessel circulation disorders, leading to inflammation, necrosis, and ulceration of the intestinal mucosa. The lesions mainly involve the rectum and sigmoid colon, and can affect the entire colon and the ileum when severe.

All Shigella bacteria produce endotoxins and exotoxins. Endotoxins can cause systemic reactions such as fever, toxemia, septic shock, and important organ failure. Exotoxins have enterotoxins, neurotoxins, and cytotoxins that cause corresponding clinical symptoms.

2. Source of infection

Sources of infection include patients and carriers. Patients with mild atypical psoriasis and chronic occult bacillary dysentery were important sources of infection.

3. Transmission route

Shigella is excreted with the feces of patients or carriers and spread through contaminated hands, food, water or living contacts, or indirect methods such as flies and cockroaches, and eventually enters the digestive tract to cause susceptible individuals.

4. Crowd susceptibility

The population is generally susceptible to dysentery bacillus. Preschool children suffer from more illnesses and are related to bad health habits. The adult patients have lower body resistance and more chances of contact infection. In addition, there is no consolidation of immunity after suffering from the same type of dysentery, and between different groups of bacteria. There is no cross-immunity between different serotypes of Shigella so it causes repeated infections or re-infections with repeated attacks.
